",{"type":15,"attrs":397,"content":398},{"textAlign":53},[399],{"text":400,"type":301}," “HEC’s involvement has been a major benefit,” says Susanna Wong, Knowledge Translation and Implementation Specialist for the ASKmeGOC team at Royal Victoria Regional Health Centre (RVH) in Barrie, Ontario. “Their connections to researchers, thought leaders and others in the system advances the work we’re doing – to spread and scale the adoption of ASKmeGOC across Ontario.” ",{"type":15,"attrs":402,"content":403},{"textAlign":53},[404],{"text":405,"type":301}," Three critical care physicians at RVH developed a web-based tool called ASKmeGOC that supports structured goals-of-care conversations with patients and their substitute decision-makers. These conversations focus on what matters most to a person – their values, priorities, and preferences for care. In Canada, physicians are typically responsible for conducting them, spending 10 to 20 minutes per patient. Across a healthcare system, that can translate into thousands of physician hours each year.  ",{"type":15,"attrs":412,"content":413},{"textAlign":53},[414],{"text":415,"type":301}," ASKmeGOC empowers other trained members of the care team, such as nurses, physician assistants and allied health professionals, to lead these conversations. A one-year research study conducted at RVH, where a team of nurses used ASKmeGOC to guide goals of care discussions, showed reduced physician workload and improved experiences for patients, families and care teams. But technology alone does not guarantee change. Experience across the program shows that even the best tools need the right conditions to succeed. Without attention to workflow, culture and adoption, new technology can simply add another layer of work instead of the intended value. Teams are supported to test, adapt and refine their solutions in real time.
