Safer care doesn’t happen by chance. It is something we create together.
Canadian Patient Safety Week (CPSW) 2025 has come to a close, and we are filled with gratitude for everyone who added their voice to this year’s campaign. From clinicians and patient partners to community organizations and health leaders, thousands joined the conversation to make care safer for all.
This year marked the 20th anniversary of CPSW, a meaningful milestone that invited reflection, learning and renewed commitment. Under the theme All Voices for Safer Care, we celebrated the power of listening, respect, trust and collaboration. We also worked to centre voices that have not always been heard, especially those most affected by health inequities.
Listening, learning and amplifying voices
Throughout the campaign, we heard from people across the country who shared how safety shows up in their workplaces, communities and personal experiences.
" Everyone contributes to promoting safer care from patients, to different roles, departments, community and patients and families.” -CPSW Participant
Organizations hosted in-person and virtual events that explored what “feeling safe” means beyond the absence of harm. Together we saw that safety includes cultural, psychological and emotional well-being and that improvement happens when every voice is valued.
Stories that inspire change
Storytelling was at the heart of this year’s campaign. We launched a new “All Voices for Safer Care” blog series highlighting champions from across Canada who are re-imagining what safer care can look like in their settings. These stories showed that change does not always come from large-scale initiatives. It often starts with one conversation, one reflection, or one small act that brings voices together for safer care.
We heard about organizations building trust through open dialogue, teams using CPSW resources to spark conversation and individuals using their lived experiences to shape better systems.
Across sectors from hospitals to long-term care, community health and administration, participants reminded us that safety is everyone’s responsibility and that improvements begin to happen when all voices are heard.
