Care Forward is a pan-Canadian movement of healthcare changemakers sharing knowledge and applying proven approaches to make healthcare better for more people. With support from Healthcare Excellence Canada—through funding, resources, expert coaching and a network of peers—they are driving lasting local impact and improving care for at least one million people across the country.
We’re focused on driving change on four key priorities, each one supported by a suite of offerings and resources that help healthcare teams make meaningful improvements:
Right Care Challenge
Open to health and social care teams ensuring people get the right care, at the right time, in the right place.
Enhancing Integrated Care
Open to organizations working to strengthen integrated team-based care, including virtual care.
Primary Care Access Improvement
Offers team-based primary care organizations opportunities to create efficiencies and optimize team functioning so they can provide timely access to care.
Paramedics and Social Prescribing
Supports paramedic teams to use social prescribing to connect clients with local community services.
Nursing Home Without Walls
Supports jurisdictions across the country to bring nursing home support and services to older adults in their own homes, helping them age safely and comfortably where they already live.
Sparking Change in the Appropriate Use of Antipsychotics
Provides long-term care homes with support to use person-centred approaches to reduce potentially inappropriate antipsychotic use.
