Nursing Home Without Walls (NHWW) is a community-based model that helps older adults age where they call home by connecting them with health, social and community supports. The model has expanded to 39 nursing homes in New Brunswick and is now being spread across six additional jurisdictions in Canada, with support from Healthcare Excellence Canada and funding from Waltons Trust.

As NHWW expands into new communities, partnerships with healthcare providers, social services and community organizations are essential to its success. These relationships help teams build on existing services and local expertise to reach more older adults and respond to the needs of their communities.

Join this webinar to explore the role of partners in the NHWW model and hear examples of how collaboration has helped increase the use of existing services, reduce duplication and strengthen community resources that support aging in place.
